What is Lymphedema

Lymphedema is a chronic condition that causes swelling when the lymphatic system cannot properly drain lymph fluid from the body's tissues. The swelling most commonly affects the arms or legs but can also occur in the hands, feet, trunk, head, neck, or genital area.

Without proper treatment, lymphedema can progress over time, leading to increased swelling, skin changes, discomfort, reduced mobility, and a higher risk of infection. While there is no cure, lymphedema can be successfully managed with early intervention and consistent care.

What are the Different Types of Lymphedema?

Phlebolymphedema

The most common condition we treat. Phlebolymphedema develops when chronic venous insufficiency (poor circulation in the veins) overwhelms the lymphatic system. Over time, the lymphatic vessels become damaged, causing persistent swelling.

Common signs:

▫️ Swelling in the lower legs and feet

▫️ Heaviness or aching

▫️ Skin discoloration

▫️ Thickened or hardened skin

▫️ Venous ulcers or slow-healing wounds

Obesity-Related Lymphedema

Excess body weight places additional stress on the lymphatic system, making it difficult for lymph fluid to circulate properly. This often causes swelling in the legs, abdomen, or other areas.

Common signs:

▫️ Persistent swelling

▫️ Difficulty moving

▫️ Skin folds with moisture buildup

▫️ Increased risk of infection

Cancer-Related Lymphedema

Cancer treatments such as surgery, lymph node removal, or radiation therapy can damage the lymphatic system, leading to swelling months or even years after treatment.

Common signs:

▫️ Swelling of an arm or leg

▫️ Tightness or heaviness

▫️ Decreased flexibility

▫️ Clothing or jewelry fitting tighter

Lipedema

Lipedema is a chronic fat disorder—not lymphedema—but it is often mistaken for it. It causes a symmetrical buildup of painful fatty tissue, usually in the legs and sometimes the arms. Some individuals may develop both lipedema and lymphedema (called lipo-lymphedema).

Common signs:

▫️ Painful fat deposits

▫️ Easy bruising

▫️ Swelling that typically spares the feet

▫️ Legs larger than the upper body

How is Lymphedema Treated?

Treatment focuses on reducing swelling, improving mobility, protecting the skin, and preventing complications.

Your therapy plan may include:

▫️ Complete Decongestive Therapy (CDT) – the gold standard treatment

▫️ Manual Lymphatic Drainage (MLD) – a gentle hands-on technique that encourages lymph fluid to move

▫️ Compression bandaging or garments to control swelling

▫️ Therapeutic exercises to improve circulation

▫️ Skin and wound care to reduce infection risk

▫️ Education and home management strategies

What to Expect During Your First Visit

During your initial evaluation, your therapist will:

✔ Discuss your goals and symptoms

✔ Develop an individualized treatment plan

✔ Teach you ways to manage swelling at home

Compression Garments

Compression garments help prevent fluid from returning after treatment and are an important part of long-term lymphedema management.

Your therapist will help determine:

▫️ The best garment style

▫️ Proper fit and wear schedule

▫️ When garments should be replaced

Home Care Tips

You play an important role in managing your lymphedema every day.

✔ Keep your skin clean and moisturized.

✔ Wear your compression garment as instructed.

✔ Stay active with gentle exercise.

✔ Elevate the affected limb when appropriate.

✔ Maintain a healthy weight.

✔ Avoid cuts, burns, or insect bites when possible.

✔ Protect your skin from injury.

✔ Follow your therapist's recommendations consistently.

When Should I Call My Therapist or Physician?

Contact your healthcare provider if you notice:

▫️ Sudden or rapidly increasing swelling

▫️ Redness, warmth, or tenderness

▫️ Fever or chills

▫️ New or worsening pain

▫️ Open wounds or drainage

▫️ Skin changes or blisters

▫️ Compression garments that no longer fit properly

▫️ Signs of infection such as cellulitis

Seek immediate medical attention if you experience chest pain, difficulty breathing, or sudden severe swelling.

Can lymphedema be cured?

No, but it can be effectively managed with proper treatment and lifelong self-care.

Will exercise make my swelling worse?

No. Gentle, guided exercise often improves lymphatic flow and reduces swelling.

Do I need to wear compression forever?

Many patients benefit from long-term compression to maintain treatment results. Your therapist will determine what's appropriate for you.

Is swelling normal after cancer treatment?

Some swelling can occur after surgery or radiation. Persistent swelling should always be evaluated.
