Getting started with Lymph At Home is simple. Here’s what to expect:
1. Get a referral from your doctor
Your doctor will need to provide a referral for the therapy or services you need.
2. We review your referral and insurance
Once we receive your referral, our team will review the information and verify your insurance benefits. Lymph At Home is an out-of-network provider, so we will review your out-of-network benefits and help you understand what to expect.
3. We’ll call you to confirm your information
A member of our team will contact you to confirm the information provided on your referral. We’ll also collect any missing details, such as your current address, email address, secondary insurance, or other important information.
4. We’ll check for other home health services
We’ll ask whether you are currently receiving home health, therapy, nursing, or other services at home. This helps us make sure our services can be provided appropriately and do not conflict with your current care.
5. Complete your Lymph At Home intake form
We’ll send you an intake form to complete before your evaluation. This gives your therapist important information about your health, history, and current needs so we can prepare for your first visit.
6. Schedule your evaluation
Once the necessary information and paperwork are completed, our therapists will work with you to schedule your evaluation and first visit.
Your first visit is the beginning of a personalized plan of care designed around your needs and goals.
